Request a test myself

Canterbury SCL recommend that all laboratory testing is ordered via the patient’s healthcare provider, however we recognise there are some instances where patients may need to order their own laboratory tests. In these instances patients are able to order from a limited test range.

Patients must be aged 16 or over to self request a test.

Making a request

Self Request forms are available at all of our collection centres.

Patients will need to enter name, address and date of birth, and the test being requested.

The following tests are not available for self request:

  • HIV
  • Genetic and Paternity Tests
  • Any Cytology
  • Any Histology

Results

Patients can have the results either emailed or mailed to a nominated address. Canterbury SCL takes no responsibility for the security of emailed results.

Please note that your healthcare provider will not receive a copy of your results.

Patients will need to consult their healthcare provider for all interpretive advice about results.

Making payment

Payment must be made at the time the sample is collected. Payment can be made by EFTPOS or Credit Card.

Note: No payments will be accepted within 30 mins of closing.
